Improving Hip Fracture Care: Learning from Data and Each Other

Seven regional hospitals belonging to SAZ, a network of collaborating general hospitals, participated in the network's Value-Based Healthcare Programme on hip fracture care. LOGEX supported this initiative by providing data-driven insights, enabling healthcare professionals to reflect on their own practices and learn from one another. 

The challenge

Hip fracture care is intensive and complex. The patient group is vulnerable; hospital stays are often lengthy, and there is a significant risk of complications or even mortality. During the programme, it became clear that there were variations in clinical practice between hospitals. For example, osteoporosisdespite its link to increased risk of repeat fractures and deathdoes not yet receive consistent attention across all hip fracture care pathways.

Our solution

LOGEX combined clinical outcome data with financial and treatment pathway data, presenting it in an open benchmarking dashboard. This level of transparency and peer comparison is rare in practice. 

Three in-person reflection sessions were held, where multidisciplinary improvement teams, including doctors, nurses, physician associates, and project leads, discussed their own data alongside that of other hospitals. Geriatric medicine specialists also joined the sessions, adding valuable perspectives. Discussions covered the importance of setting clear expectations and building mutual trust across the care pathway. 

This combination of data and open dialogue enabled critical reflection and the sharing of best practices. For example, one hospital improved osteoporosis screening rates by following up DEXA scan invitations with phone calls. Others discussed the benefits of administering zoledronic acid to slow bone loss and reduce fracture risk. Some participants shared positive experiences with initiating discharge planning early in collaboration with community care partners. There was also debate around how best to approach conservative treatment options for hip fractures.

Our Impact

Adjustments based on lessons learned were made during the programme. One hospital discovered through benchmarking that its average length of stay was higher than others. By reviewing emergency department and inpatient processes and strengthening collaboration with geriatric specialists and other care partners, they successfully reduced the length of stay.
