About Kanta-Häme

Kanta-Häme is a mid-sized wellbeing services county in Finland, responsible for delivering integrated healthcare and social services to its population. With a strong strategic focus on customer-centricity, the organisation aims to improve outcomes by understanding and supporting individuals across their entire care journey.

The challenge

Like many healthcare systems, Kanta-Häme faced the challenge of managing services across organisational silos, with limited visibility into how individuals move through the system. While data was available within specific domains, such as primary care or specialised healthcare, it was difficult to gain a holistic view of patient pathways.

Another challenge connected to the data silos, was the inability to link outcomes to resource use. Without cost insights, it remained challenging to assess cost-effectiveness, prioritise improvements, and compare different service models.

The solution

To address this, Kanta-Häme developed a person-centred data model that brings together data across healthcare and social services. This model creates visibility into individual care pathways and how people use services over time.

In collaboration with LOGEX, the organisation is extending this model with LOGEX Costing, enabling patient-level cost accounting across the full care journey.

LOGEX Costing connects financial data to individual pathways, making it possible to:
  • Understand how resources are used across services
  • Compare different activities using a consistent unit
  • Link costs directly to outcomes
By combining pathway data with cost insights, the organisation can use the model to support decisions, rather than analysis alone.

The impact

The addition of LOGEX Costing allows Kanta-Häme to assess both outcomes and resource use in a single view. This supports more informed decisions across the organisation.

Leadership teams compare different approaches to care, identify where resources concentrate, and determine where changes are likely to have the greatest effect. The model also provides context for variation across services.

As coverage expands across the organisation, the model creates a more consistent basis for prioritisation. This helps Kanta-Häme improve efficiency and outcomes while maintaining a clear view of how resources are used across the system.
